The Auburndale and Lakeland health clinics provide adults with comprehensive screenings and preventive exams. Staff at both clinics evaluate and treat minor illnesses (fever, coughs, and colds) and chronic diseases, such as asthma, high blood pressure, and diabetes.
No one will be denied access to services due to inability to pay, and there is a discounted/sliding fee schedule available. Uninsured adults may be eligible to receive services for free under the Polk community health care program. Contact us for more information about eligibility.
The Polk HealthCare Plan is a partnership between the Florida Department of Health in Polk County (DOH-Polk) and the Polk County Community Health Care Division. This program provides no-cost primary medical and dental care to uninsured Polk County residents who meet certain income requirements.
