Health Department Provides Back-to-School Immunizations

It’s back-to-school time again! For most students, especially those entering kindergarten, seventh grade, or new to Polk County schools, this means a visit to the doctor for school immunizations. The Florida Department of Health in Polk County (DOH Polk) is helping parents meet the requirements for back-to-school immunizations by offering extended clinic hours.

Students are required to have all mandated vaccines to attend Polk County schools. If you have children who need the required immunizations, staff at the Florida Department of Health will provide additional services during the following dates:

  • Monday, August 17 to Wednesday, August 26, 2015
    • Immunizations offered 7:30 a.m. to 5:00 p.m. at Auburndale, Haines City and Lake Wales clinics.
    • Immunizations offered 7:00 a.m. to 5:30 p.m. at Lakeland clinic.
    • Addresses and phone numbers can be found on the DOH-Polk website at https://polk.floridahealth.gov/locations/index.html.
  • Wednesday, August 12 and 19- “Back-to-School Physicals” at Lakeland Clinic
  • Physicals offered 1:00 p.m. to 4:30 p.m. at the Lakeland Clinic and ONLY for:
    • Children entering school for the first time (Pre-K or Kindergarten) and who do NOT have proper physical signed and dated for the last 12 months.
    • Children entering Florida schools from out of state or out of county and who do NOT have proper physical signed and dated for the last 12 months.
  • No appointment needed, walk-ins welcome. First come, first serve.

Tdap vaccines are mandatory for student’s entry into 7th grade. The Florida Department of Health in Polk County will be offering this free vaccine for any rising 7th grader at the following times and locations:

  • Thursday, August 20, 2015– “Tdap Clinic”
    • Southwest Middle School (Lakeland)- 9:00 a.m. to noon
    • Denison Middle School (Winter Haven)- 9:00 a.m. to noon
  • McLaughlin Middle School (Lake Wales)- 5:00 p.m. to 7:00 p.m.

A parent or guardian must come with children who need to be vaccinated.  Please bring current shot records with you, if you have them.
