Some Services Temporarily Unavailable as FDOH-Polk Upgrades Equipment

BARTOW - The Florida Department of Health in Polk County will be undergoing equipment upgrades May 18- June 1 at various site locations. As a result, computers and telephones at these locations will be temporarily unavailable and our clinic services will be limited. We apologize for this inconvenience, however, staff will be present at all site locations to answer any questions individuals may have or refer them to alternate clinic locations for services.

The following schedule shares what clinical services will be limited on its corresponding date:

  • Monday, May 18- Auburndale Clinic
  • Monday, May 18- Auburndale Clinic- Tuberculosis Control Clinic
  • Tuesday, May 19- Lake Wales Clinic & WIC
  • Wednesday, May 20- Winter Haven Radiological Health
  • Thursday, May 21- Winter Haven Environmental Health
  • Thursday, May 21- Winter Haven WIC
  • Friday, May 22- Haines City Clinic
  • Tuesday, May 26- Bartow Environmental Support
  • Tuesday, May 26- Bartow Dental Clinic
  • Wednesday, May 27- Bartow Healthy Start and School Health
  • Thursday, May 28- Bartow Specialty Care Clinic
  • Monday, June 1- Lakeland Clinic (which includes Dental, WIC and Vital Statistics)
