DOH-POLK TB CONTROL CLINIC AND REFUGEE HEALTH IN AUBURNDALE TO RELOCATE

Bartow, Fla. The Florida Department of Health in Polk County (DOH-Polk) is pleased to announce the relocation of our TB Control Clinic and Refugee Health Program from Auburndale to our new location in Bartow. The new location is:

Bartow Medical Annex
2020 E. Georgia St.
Bartow, FL 33830

The Bartow clinic will open to the public on Tuesday, January 9, 2024. Business hours are 8 a.m. to 5 p.m. Monday through Friday. The phone number for the new clinic, effective Tuesday, January 9, 2024, is 863-578-2290.

The Auburndale clinic will close from Friday, January 5, 2024, through Monday, January 8, 2024.   

If you have any questions or concerns about appointments or walk-ins during this transition period, please call 863-519-7900 to speak to a representative.
