Florida SHOTS (link opens in new window) is a free, statewide, centralized online immunization registry that helps health care providers, parents, and schools keep track of immunization records. You can also visit ImmunizeFlorida.org to learn more about immunizations your child may need.

Immunization services for adults, including travel immunizations, will be temporarily suspended at all DOH-Polk locations until August 15, 2022. Please contact your primary care provider or local pharmacy for adult vaccines. Travel vaccines can be scheduled through Passport Health (link opens in new window).
