The Florida Department of Health in Polk County utilizes community data and information to deliver services and programs that improve the health of our community. One of the responsibilities of DOH-Polk is to conduct community health assessments and provide data and information related to the health of the community.

Florida’s Community Health Assessment Resource Tool (CHARTS) is a one-stop website for public health statistics and community health data in Florida. It provides access to a wide range of information, including health statistics like births, deaths, disease rates, population data, and behavioral risk factors
State Health Improvement Plan
The Florida State Health Improvement Plan sets out goals for Florida’s public health system, which includes a range of stakeholders, such as state and local government agencies, health care providers, employers, community groups, universities and schools, nonprofit organizations, and advocacy groups.
