One of the responsibilities of the health department is to conduct community health assessments and provide data and information related to the health of the community. The process that the Florida Department of Health in Polk County (DPH-Polk) uses to conduct community health assessments is known as MAPP, which stands for Mobilizing for Action Through Planning and Partnerships.
The data collected through the MAPP process is analyzed and compiled into what is known as the Community Health Assessment (CHA). This document, which is produced every five years, identifies factors that determine the availability of resources within the community that can be mobilized to improve the health status of Polk County. It assures that local resources are directed toward activities and interventions that address critical and timely public health needs.

The results of the Community Health Assessment are used to create a Community Health Improvement Plan (CHIP). The CHIP is a community-wide strategic plan to improve the health and quality of life of the people who live in Polk County. Upon completion of the 2020 Polk County Community Health Assessment, community partners worked together to create goals and objectives for the 2021-2025 Polk County Community Health Improvement Plan (CHIP). Each year, the CHIP is reviewed, and revisions are made as needed.
